COVID-19 Staff Forms

COVID-19 Testing Confirmation Submission (alternate test location)
This form is completed by staff who have identified themselves as not vaccinated or those who have not responded to the COVID-19 vaccination status survey. This form must be completed every week during the testing process for the 2021-22 school year.

UPDATED: COVID-19 Vaccination Status Survey
Use this form to submit updated information if you received your booster shot, if you never submitted your vaccination status, or if your employment started after Jan. 01, 2022.

Employee COVID-19 Form
This form is completed if staff tests positive for COVID-19, has been in contact with someone who tested positive for COVID-19, and/or has COVID-19 symptoms. You will receive a call from HR or the nursing staff within 24 hours of completing the form.

Weekly Testing COVID HIPPA Form
Complete the HIPPA form if you will be participating in the district's weekly COVID-19 testing program. This form only needs to be completed online once.
